Support Coordination and Connection Services
Help understanding and implementing the NDIS plan, shortlisting and connecting to suitable
providers, tracking goals, troubleshooting service gaps, and navigating changes when life shifts.
This is our core service across Western Australia.
Who it’s for Participants and families in WA who want a trusted coordinator — not a roster of support workers from us.
Provider and mainstream connection
We research, shortlist, and help you engage NDIS providers and mainstream services
(for example daily living, community participation, therapy, or other funded supports).
Those organisations deliver the hands-on support; we make sure the connections fit your goals and preferences.
Who it’s for People who need the right providers found and linked — not Felix SCC staff attending as support workers.
Capacity building & plan literacy
Support to build skills in choice and control: understanding the plan, using the NDIS portal where relevant,
preparing for meetings, and advocating for yourself with providers and the Agency.
Who it’s for Participants who want to grow independence in navigating the system.
Family & carer partnership
We include families and carers in the coordination conversation — explaining options,
reducing overwhelm, and aligning everyone around the participant’s goals —
while keeping decision-making with the participant wherever possible.
Who it’s for Families and carers across Western Australia who want clarity and a reliable coordination partner.
What we do not provide
Felix SCC does not supply support workers, personal care shifts, SIL staffing, or other
direct “core supports” delivery. If your plan needs those services, we help you connect
to appropriate providers as part of Support Coordination and Connection Services.
Need workers? Ask us — we will help you find and onboard a suitable provider.
