Support Coordination and Connection Services
Plan implementation, provider shortlisting, and ongoing coordination so participants across Western Australia can exercise choice and control with a clear guide beside them.
Felix SCC
Felix SCC delivers NDIS Support Coordination and Connection Services across Perth and WA — helping participants and families understand their plan, choose providers, and stay connected to the right services. We coordinate and connect; we do not provide support workers.
Support Coordination and Connection Services — not direct care or support-worker shifts.
Plan implementation, provider shortlisting, and ongoing coordination so participants across Western Australia can exercise choice and control with a clear guide beside them.
We help you find and connect with suitable NDIS and mainstream providers — including daily living, community, therapy, or other supports funded in the plan. Those providers deliver the hands-on support; we make the connections work.
Portal literacy, self-advocacy, and plan understanding — so over time you need less “doing for” and more confident choice of your own.
We include families and carers in the coordination conversation — explaining options, reducing overwhelm, and keeping the participant’s goals at the centre.
Felix SCC is led by experienced practitioners who know the sector from the inside. Our COO, Sukhi Tear, previously founded Diversitas WA — bringing practical Support Coordination and Connection Services leadership into a warm, participant-centred company.
Based in Perth and serving Western Australia, Australia, we focus on relationships that last — not paperwork for its own sake.
Our storyBehind every conversation, we use careful systems so funding and documentation stay sound — without getting in the way of Support Coordination and Connection Services in WA. How we work
